Practical Applications for a Variety of Needs

Create a customized assessment to meet the specific needs of the individual, program and support team.

Planning for College

Too often families assume academic success in high school predicts academic success in college. While academic strength certainly helps, what often gets missed is the interdependent nature of college and the premium placed on these skills. Self-advocacy, sleep hygiene, diet/nutrition, social navigation, stress management, etc. are all skills necessary for success in college, and are intertwined with academic success.

The P2I Assessment system allows you to choose the right assessments tailored to the individual to help determine strengths and areas of deficiency, prior to the start of college, allowing for more effective transition planning.

IEP Transition Planning

IEP transition planning requirements include developing measurable post-school goals in the areas of post-secondary education/training, employment, and independent living, starting no later than age 16. The plan must include student preferences and interests, and specify the transition services needed to achieve these goals.

The P2I Assessment System was specifically designed to address the needs of students who learn differently. Whether identified as a student with a Specific Learning Disability (SLD), Autism (ASD), or Other Health Impairment (OHI), the P2I Assessment System provides IEP teams with the necessary information to create a thoughtful, targeted, and effective transition plan with a clear roadmap to independence.

Treatment Planning

One of the key outcomes for all treatment programs is to help their clients achieve their maximum level of independence, but determining transition readiness can be complex and challenging.

The P2I Assessment System can help to evaluate a client’s ability to monitor their own physical and mental health, manage their finances and daily tasks, and maintain and develop personal relationships – all necessary components of a successful and independent adult lifestyle.

Combine assessments to create a comprehensive Adaptive Life and Social Emotional Skills analysis of strengths and areas of need, which can be used in treatment planning and progress monitoring. By quantifying and measuring critical skills, programs are able to improve individual and programmatic outcomes.

Entering the Workforce

Whether going to work after high school or college, the transition to the workplace can be equally complex, requiring a slightly different set of skills.

Managing finances, connecting with community, and possessing critical employability skills are essential. Being smart consumers of online content, collaborating with a team, compromising on issues of disagreement, taking initiative and demonstrating leadership are just some of the skills necessary for success in any field.

The P2I Assessment System can provide you with a comprehensive evaluation on of the skills one needs to successfully transition to independent living and a vocation, whether that transition occurs after high school or college.
